OWNER'S REGISTRATION FORM

Dear Terex South Dakota, Inc. Equipment Owner(s):

Once Submitted, there are two (2) registration cards.

Lower Card is to be completed and sent to Terex South Dakota, Inc. by the original owner or by the Selling distributor if unit is a Demo within 10 days of delivery of the equipment by the Terex South Dakota, Inc. dealer, to you. Upon receipt of this information your Terex South Dakota, Inc. warranty becomes valid. If the unit is in demo status, returning the card will serve as unit registration to allow warranty claim processing.

Upper card is to remain in this manual until the Terex South Dakota, Inc. unit and Operator’s, Maintenance, and Service Parts manuals are placed into the possession of the second owner or sold to owner if unit was in Demo status. At this time, the warranty information should be completed and sent to Terex South Dakota, Inc. to update our ownership records. The original owner is responsible for notifying the second owner, and the Demo Dealer is responsible for notifying the sold to owner of the registration requirement to insure subsequent communications, if any, are received by the subsequent owner(s).
